RN Case Manager: Elevate Care Coordination & Outcomes

Baptist Health is seeking an RN Case Manager to support physicians and the interdisciplinary team in coordinating patient care, discharge planning, and cost-effective resource use. You will manage a designated patient caseload and collaborate with UR RN and Social Work Case Manager to improve outcomes. Minimum 3 years hospital clinical experience; RN license required; BSN within 3 years of entry.
