Responsibilities
- Assess, plan, implement, and coordinate care for SCU and Skilled Nursing/Extended care patients
- Administer medications and treatments per orders; monitor patient status and document findings
- Collaborate with RNs and the broader care team; communicate changes in patient condition promptly
- Provide patient and family education; assist with discharge planning and continuity of care
- Maintain safety, infection control, and adherence to facility policies and procedures
- Participate in on-call coverage as required; respond to patient needs during night shifts and escalate as needed

Requirements
- Current LPN license (state verification per job notes)
- Ability to work night shifts, 12-hour blocks
- On-call availability required (2 shifts per 6 weeks)
- Experience or comfort providing care in a skilled nursing/extended care setting
- Willingness to travel for a temporary assignment; 13-week contract length
- Ability to work in a hospital-based, longer-term care environment
