Key Responsibilities\n

• Conduct comprehensive psychosocial assessments to identify client strengths and areas of need, employing standardized assessment tools to ensure thorough evaluations.

• Provide evidence-based therapy interventions, both individually and in group settings, utilizing modalities such as Cognitive Behavioral Therapy (CBT) and Solution-Focused Therapy.

• Work collaboratively with a diverse interdisciplinary team, including physicians, nurses, and other mental health professionals, to create holistic care plans tailored to the client's diagnosis and needs.

• Advocate for clients to access community resources, including financial support services, counseling, and healthcare systems, enhancing their access to comprehensive care.

• Maintain accurate and timely documentation of client interactions and treatment progress using the latest electronic health record (EHR) systems.

• Engage in regular professional training and team discussions, staying current with best practices in social work, hospice care, and therapeutic techniques.

Qualifications\n

• A Master’s degree in Social Work (MSW) from an accredited institution is required.

• Valid and current licensure as a Licensed Clinical Social Worker (LCSW) in the state of California.

• Proven experience in social work, specifically working with older adults and in hospice settings.

• Strong clinical assessment and diagnostic skills, coupled with the ability to foster a therapeutic relationship with clients.

Compensation and Benefits\n

We offer a competitive salary ranging from $110,000 to $130,000 annually, commensurate with experience.
