Job DetailsSchedule: Sunday-Thursday 9:00am-5:00pm Summary Monitors the health, safety and well being of participants in the Residential system. Incumbent is responsible for overseeing and managing the medical and health care of program participants including providing direct nursing care. Incumbent reports issues to appropriate persons and handles emergencies according to Incompass Human Services and other applicable agencies guidelines. Incumbent must comply with applicable regulatory documentation and standards in the performance of nursing duties. This position meets the regulatory requirements set forth in 130 CMR 419.426(G) for Licensed Practical Nurses and is directly supervised by an RN as required. This is a bargaining unit position. Essential Job Functions Monitors the health of program participants, including preventative care, and provides professional technical assistance regarding nursing care and medical treatment as needed. Responsible for management and documentation as it relates to medical information, referrals, reports, medication administration and data collection for use in annual planning, goal setting and intervention strategies. Responsible for the completion of medical record audits and management of medical tracking systems. Assesses individuals as assigned on a regular basis and provides management for individuals with major medical issues. Provides oversight and assistance with the self-medication assessment process. Provides oversight of MAP and for the administration of medical treatments and staff training as needed including annual mandated staff trainings. Administration is done in accordance with Nurse Practioner’s Act and DDS and MAP regulations. Provides assistance when needed regarding medication supplies and medical supplies as needed. Orients and trains staff in understanding participants’ medical needs and in following prescribed programs and plans. Communicates with and trains staff to complete procedures as appropriate. Provides leadership and support to team during medical crisis. Ensures professional communication with families, DSS, clinicians and others for assigned participants. Provides guidance to staff regarding medical emergencies and informs supervisor of all emergency treatments or 911 calls made. Communicates with medical providers regarding major issues. Completes nursing notes and documentation for encounters with individuals. Attends meetings as assigned Ensures the coordination between Residential and Day Habilitation program for continuity of care. Serves as a mandated reporter as required by the Disabled Persons Protection Commission. Processes all human rights issues, incident and accident reports in a timely manner according to agency policy/procedure. Ensures the human rights of program participants are safeguarded in all interventions. Responsible for knowing and following agency, departmental, program and site policies and procedures as well as the bargaining unit personnel policies handbook and the collective bargaining agreement. Fosters and maintains positive relationships with families, participants and staff and informs supervisor of major and/or unresolved issues. Acts in a manner consistent with organizational values, program mission, and service delivery goals. Serves as a resource and role model for staff. Completes other duties as assigned by supervisor as they relate to the duties in this job description. Education and Experience Must be a Licensed Practical Nurse registered with the MA Board of Registration in Nursing At least one year of experience working with the elderly, children, or persons with developmental disabilities. Strong interpersonal, written, and verbal communication skills required Valid drivers license Supervisory Responsibilities None Analytical Skills Demonstrated ability to problem-solve, handle emergencies, and successfully manage several projects simultaneously. Language Skills Mastery of the English language required as demonstrated by the ability to read and write instructions, ability to read, analyze, and interpret field and/or professional journals, technical procedures, policy and procedure manuals and governmental regulations. Strong interpersonal skills and the ability to respond to common inquiries or complaints from Individuals, families, staff or the general public Strong Ability to comprehend and follow verbal/written instructions. Ability to write reports, business correspondence, and procedures. Certificates, Licenses, Registrations Current Adult CPR certification. PAC or other form of current agency approved restraint certification must be acquired during the first six months on the job. Employee is required to remain current with Adult CPR certification, PAC/restraint certification and LPN license. Nurse must maintain a clean record and not have any pending discipline actions with the Commonwealth of Massachusetts or job status will be reviewed with the outcome to be decided by a management team.
