Description

American Traveler is seeking a Home Health Physical Therapist with at least 1 year of home health experience to provide in-home patient care across multiple service areas.

Details

• Home health care setting covering Clarendon, Sumter, Lee, Darlington, and Florence counties
• Schedule is 08:00-17:00 with an expectation of at least 20 patient care units per week
• Services include routine PT visits, start of care, evaluations, reassessments, supervisory visits, OASIS assessments, discharges, and transfers
• Limited time off requests, not to exceed 2 consecutive days

Requirements

• Minimum 1 year of Home Health experience required
• Minimum 1 year of Physical Therapy experience required
• Current BLS certification required (AHA only)
• License required if applicable
• Must have strong manual dexterity for clinical and technical care tasks
• Good hearing and eyesight required
• Ability to perform moderate physical activity including walking, standing, bending, lifting, and carrying up to 50 lbs
• Ability to drive to patient homes across multiple service areas

Additional Information

• Provides physical therapy services in accordance with physician orders and agency policies
• Interacts daily with patients, families, and agency personnel, and regularly with physicians
• Functions independently while managing time, attendance, and reporting requirements
• May serve as a clinical education coordinator, mentor, or preceptor for students and volunteers
• Requires knowledge of state therapy laws and Medicare Conditions of Participation for Home Health Agencies
• Strong interpersonal, communication, and conflict resolution skills required
• Must demonstrate professionalism, self-motivation, and sound clinical judgment
• Open to first-time travelers
