Responsibilities
- Deliver direct nursing care to adults and geriatric residents, including medication administration (IM/SQ injections, basic IV/med administration) and monitoring responses.
- Perform activities of daily living support, wound care, Foley care, NG/G-tube care, and basic dressing changes; support feeding and hydration needs.
- Monitor vital signs and oxygen therapy; identify and report changes in patient status; respond to clinical concerns per unit protocols.
- Provide dementia care with behavior management and de-escalation strategies; participate in palliative and end-of-life care as appropriate.
- Document comprehensive care and interventions in EPIC; coordinate with the interdisciplinary team (PT, radiology, phlebotomy, social services, pharmacy, unit secretary) to ensure seamless care.

Requirements
- Active Licensed Practical Nurse license for New York (in hand)
- BLS certification
- EPIC charting experience (hard requirement)
- Minimum 1 year clinical experience
- COVID-19 vaccine: Not required; Flu vaccine: Unknown
- Ability to work in a 42-bed long-term care/unit setting and adhere to unit ratios and facility policies
- Orientation: 16 hours provided prior to start
