About the Role:
As a Licensed Practical Nurse (LPN), you will play a vital role in providing high-quality patient care to our residents and patients. Your duties will include administering medications, performing wound care, and assisting with daily living activities. You will work collaboratively with our interdisciplinary team to develop and implement individualized care plans. Your compassion, empathy, and strong communication skills will enable you to build meaningful relationships with our residents and their families.
Responsibilities:
Administer medications and treatments as prescribed by physicians
Monitor and report changes in medication efficacy and patient condition to physicians
Perform wound care, passive range of motion exercises, and other treatments as needed
Assist with daily living activities, including bathing, dressing, and toileting
Develop and implement individualized care plans
Collaborate with interdisciplinary team to provide comprehensive care
Document patient care in accordance with facility policies and regulations
Provide emotional support and reassurance to residents and their families
Requirements:
Current LPN license in the state of Arizona
High school diploma or equivalent required
Prior experience in long-term care or medical setting preferred
Strong communication and interpersonal skills
Ability to work effectively in a fast-paced environment
Compassionate and empathetic approach to patient care
