IRIS Care Navigator - Remote Social Services Pro
Molina Talent Acquisition
Molina Healthcare is seeking an IRIS Consultant (IC) to work remotely with Wisconsin IRIS participants. You will visit participants in their homes and communities, build strong relationships, and help people navigate the IRIS program to achieve personal goals.
Your role focuses on developing customized IRIS plans related to employment, housing, health, safety, and community engagement, while managing budgets and ensuring HIPAA compliance. Regular team interactions support program success.
