Position ResponsibilitiesAs a Nurse Practitioner or Physician Assistant, you will:Conduct comprehensive in-home Health Risk Assessments (HRAs) for Medicare Advantage members.Review medical history, medications, lifestyle, and social determinants of healthPerform physical and mental health evaluations and document findings in real-time.Provide preventive care education without the need for ongoing patient management.Travel locally within your county and surrounding areas for in-home assessments.
