Key Responsibilities\n

• Conduct comprehensive assessments of clients’ mental and emotional needs, developing tailored treatment plans that address unique circumstances.

• Collaborate extensively with a multidisciplinary team of healthcare professionals—including nurses, doctors, and therapists—to ensure a holistic approach to patient care.

• Facilitate one-on-one counseling sessions via telehealth, providing coping strategies and emotional support to clients and their families.

• Continuously evaluate and update treatment plans based on clients' progress, responding dynamically to changing needs and situations.

Patient Population / Client Focus\n

This position focuses on providing supportive counseling to terminally ill patients and their families within the hospice care paradigm. Clients may be experiencing anxiety, depression, grief, or other emotional challenges related to their conditions, and thus require compassionate interventions that respect their dignity and individual needs.

Qualifications\n

• A Master’s degree in Social Work from an accredited program is required for this role.

• Possession of a current Licensed Clinical Social Worker (LCSW) license in the state of Virginia.
