Licensed Practical Nurse

Key Job Duties and Responsibilities The Licensed Practical Nurse is to provide direct nursing care to the residents. Such care must be in accordance with current federal, state, and local standards, guidelines, and regulations that govern our facility, and as may be required by the Director of Nursing Services or Nurse Supervisor to ensure that the highest degree of quality care is maintained at all times. The following is a general list of key job duties and responsibilities in your position. The list is not to be considered inclusive and you will be required to perform other duties/responsibilities as assigned by an authorized supervisor. Duties and Responsibilities Comply with the Facility's personnel, safety, and corporate policies and procedures. Comply with the nursing department’s policies and procedures. Report witnessed or reported allegations of resident abuse/neglect immediately to the Administrator, Director of Nursing or Unit Manager. Treat residents, family members, visitors, and team members with dignity and respect. Maintain the confidentiality of all resident and family information. Report to work on time and as scheduled. Assist in the admission and discharge of residents. Assist residents in daily hygiene care including but not limited to bathing (bed bath, shower, tub), oral, ear, eye, facial, hair and nail care. Keep resident’s dry – changing clothing and linen when it becomes soiled or wet. Assist residents in bowel and bladder functions; collecting specimens when required. Assist in transporting residents to/from appointments, activity, and social programs. Assist with lifting, turning, moving, positioning, and transporting residents into and out of beds, chairs, bathtubs, wheelchairs, lifts, etc. Report and, when appropriate, document any change observed in a resident’s condition or behavior to the Charge Nurse. Assist in the care of dying residents; provide post-mortem care. Administer professional services such as catheterization, tube feedings, suction, applying and changing dressings/bandages, packs, colostomy, and drainage bags, taking blood, etc., as required. Perform lab tests, blood pressures, and other routine monitoring as required per the resident care plan. Admit, transfer, and discharge residents as required. Complete accident/incident reports as necessary. Perform administrative duties such as completing medical forms, reports, evaluations, studies, charting, etc., as necessary. Complete and file required recordkeeping forms/charts upon the resident's admission, transfer, and/or discharge. Chart nurses' notes in an informative and descriptive manner that reflects the care provided to the resident, as well as the resident's response to the care. Fill out and complete accident/incident reports. Submit to Director as required and follow established procedures. Report all discrepancies noted concerning physician's orders, diet change, charting error, etc., to the Nurse Supervisor. Perform routine charting duties as required and in accordance with established charting and documentation policies and procedures. Develop and maintain a good working rapport with inter-departmental personnel, as well as other departments within the facility to ensure that nursing services and activities can be adequately maintained to meet the needs of the residents. Complete required documentation of care and services as they are provided. Review complaints and grievances made or filed by your assigned personnel. Make appropriate reports to the Nurse Supervisor as required or as may be necessary. Follow facility's established procedures. Receive/give the nursing report upon reporting in and ending shift duty hours. Report occupational exposures to blood, body fluids, infectious materials, and hazardous chemicals in accordance with the facility's policies and procedures governing accidents and incidents. Monitor your assigned personnel to ensure that they are following established safety regulations in the use of equipment and supplies. Recommend to the Nurse Supervisor the equipment and supply needs of your unit/shift. Review care plans daily to ensure that appropriate care is being rendered. Maintain the confidentiality of all resident care information. Oversee resident needs, staff and the operations, as needed. Essential Job Duties/Physical Requirements Regular worksite attendance. Must have a valid professional license (LPN) that is in good standing with the State. Must have a valid CPR certification. Must have basic computer skills; must be able to read, write, speak, and understand the English language. Must be able to work beyond normal working hours and on weekends and holidays when necessary. Must be able to assist in the evacuation of residents during emergency situations. Must be able to perform the essential position functions of the job with, or without reasonable accommodation. Employee is required to use a transfer/gait belt or to obtain the assistance of another employee when attempting to assist residents when lifting or carrying objects in excess of 25 pounds. The employee must be aware that during the normal, routine performance of the essential functions, some of the following body movements may occur naturally, although they may not be a requirement of the job. Primary Physical Requirements: Lift up to 10 lbs: Constantly Lift 11 to 25 lbs: Frequently Lift 26 to 50 lbs: Occasionally Lift over 50 lbs: Occasionally Carry up to 10 lbs: Constantly Carry 11 to 25 lbs: Occasionally Carry 26 to 50 lbs: Rarely Carry over 50 lbs: Not required Reach above shoulder height: Frequently Reach at shoulder height: Constantly Reach below shoulder height: Constantly Push/Pull: Constantly Hand Manipulation: Grasping: Constantly Handling: Constantly Torquing: Frequently Fingering: Constantly Other Physical Considerations Twisting, Bending, Crawling, Squatting, Kneeling, Crouching, Climbing and Balancing is required. Employees may be required to operate an automobile and various equipment. Must be able to relate to and work with the ill, disabled, elderly, emotionally upset, and, at times, hostile people within the community.
