The **LVN** is responsible for delivering treatments and services in alignment with the patient’s individualized plan of care to promote safety, comfort, and optimal health outcomes.* Implement the plan of care as developed and directed by the Registered Nurse* Provide accurate, thorough, and timely clinical documentation consistent with the patient’s plan of care and agency standards* Collect assessment data and perform observations, promptly reporting changes in patient condition to the supervising RN and/or physician* Participate in the coordination of home health services by collaborating with the interdisciplinary care team to ensure seamless, patient‐centered care* Use medical equipment, supplies, and resources safely, effectively, and efficiently* Engage in ongoing professional development, maintaining clinical competency and participating in training or in‐service education as required**Requirements*** Valid CA Licensed Vocational Nurse License* Maintain a current CPR certification* 2+ years of experience working in direct patient care* Must be a licensed driver with an automobile that is insured in accordance with state or organization requirements and is in good working order**Why Join Bella Terra Home Health & Hospice of the Desert?**Bella Terra is part of the Cornerstone Group, a nationwide network of 75+ home health and hospice agencies.
