Key Responsibilities
- Patient Care & Rehabilitation: Provide prescribed occupational therapy services and skilled procedures to minimize physical disabilities and help patients reach their optimum level of independence.
- Assessment & Care Planning: Conduct initial assessments, develop and modify maintenance plans, perform 30-day reassessments, and collaborate with multidisciplinary home care personnel on care planning.
- Physician Collaboration: Consult directly with physicians regarding changes in patient condition or treatment plans.

Requirements & Qualifications
- Education & Certification:
- Graduate of an accredited Occupational Therapy program (CAHEA/AMA/AOTA).
- Passed or eligible for the AOTA National Certification Exam.
- Licensure: Current Occupational Therapist license in the state of practice.
- Experience:
- Minimum of 1 year of experience in an acute care setting required.
- 1 year of Home Health experience strongly preferred.
- Other: Access to reliable transportation.
Compensation: $30.00 - $42.00 per hour
