Home Care Registered Nurse (RN)/Office Clinical Support - Hybrid

Position Summary The Home Care Registered Nurse (RN) is a hybrid clinical position responsible for providing skilled nursing services to patients in their homes while assisting the Director of Nursing (DON) with the clinical oversight and daily operations of the agency. This role ensures the delivery of safe, high-quality, patient-centered care in compliance with Michigan regulations, agency policies, physician orders, and applicable Medicare Conditions of Participation. The ideal candidate is flexible, adaptable, and able to thrive in a fast-paced healthcare environment. This individual demonstrates sound clinical judgment, effectively manages changing priorities, and maintains a positive, patient-centered approach while balancing direct patient care with administrative and leadership responsibilities. Key Responsibilities Clinical Home Care Responsibilities Conduct comprehensive patient assessments and develop individualized plans of care. Perform skilled nursing visits in patients' homes according to physician orders. Administer medications, treatments, wound care, and other skilled nursing services. Monitor patient progress and communicate changes in condition to physicians and the interdisciplinary team. Provide patient and caregiver education to promote disease management and independence. Maintain accurate, timely, and compliant clinical documentation. Other Responsibilities Provide clinical supervision and support to field nurses, therapists, and home health aides. Review clinical documentation for accuracy, completeness, and regulatory compliance. Assist with orientation, mentoring, competency evaluations, and ongoing education of clinical staff. Participate in quality assurance and performance improvement (QAPI) activities. Support infection prevention, patient safety initiatives, and risk management. Assist with survey readiness and ensure compliance with federal, state, Medicare and accreditation standards. Qualifications Current Registered Nurse (RN) license in good standing. Valid driver's license, reliable transportation, and proof of automobile insurance. Strong clinical assessment, communication, and organizational skills. Physical Requirements Ability to travel to patient homes throughout the service area. Ability to lift up to 30 pounds and assist with patient mobility when necessary. Ability to stand, walk, bend, and sit for extended periods. Ability to use a computer and mobile technology for documentation. Preferences Minimum of 2 years of home health or home care nursing experience preferred. Knowledge of Medicare Conditions of Participation and home health regulations preferred. Proficiency with electronic medical records (EMR) systems and Microsoft Office applications. Schedule Full-Time Hybrid position combining field visits with office-based responsibilities. Flexible scheduling with occasional evening, weekend, or on-call responsibilities as needed.
