Aftercare Coordinator

Description Position Summary : The Aftercare & Visitation Coordinator is responsible for arranging and providing aftercare and/or visitation services for clients and their families and acting as a resource person between families and various community/service programs.
Requirements • K nowledge - Non-specific Bachelors' Degree and 1 year of experience in a related field preferred. • Written Communication Skills - Job requires writing skills sufficient to complete more difficult assignments, such as editing documents and materials for style and content, drafting substantive correspondence or reports, and organizing material into formats which ensure case of reading and understanding.
• Research, Processing and Handling of Information - Information is compiled or received in a prescribed form, but may vary in content according to difference in job tasks performed. Some data or information are encountered that must be categorized, reported to others, or modified to a limited extent.
• Communication Skills - Job duties and responsibilities periodically require interpreting and translating facts and information, explaining situations and issues to people and advising them of alternative or appropriate courses of action; and/or interviewing and acquiring information from others.
• Judgment and Independence of Action - effects Incumbent's Own Position.
• Job Complexity - Data and information are mostly factual, but frequently there are minor variables, uncertainties, or ambiguities to consider. Analytical ability, conceptual ability, logic, and intuitive judgment must be applied to a limited extent. Steps, methods or processes used are normally established or pre-defined, but occasionally require some modification or adaptation.
• Impact on Client, Community and Staff Relations - The job's responsibilities are expected to have a significant impact on the image of the organization, client, community and/or staff relations can be affected to the extent that issues or problems are considered significant. Positive or negative consequences are likely to become widely known (internally and externally) and materially affect the reputation of the organization.
• Impact on Programs, Operations and Services - The job impacts the overall efficiency of the organization's programs, operations and services to a minor degree. The structure or design of the job permits or encourages actions which can improve individual or group performance. Errors or deficiencies in performance are more difficult to correct. Correction requires definite amounts of time, effort, and/or financial resources.
• Supervisory or Managerial Responsibility - No supervisory or managerial authority.
• Demand for Mental and/or Visual Concentration - Level of Visual Concentration: The amount and extent of task detail with which the person must work, e.g., figures, paperwork, data, etc are periodic. Interruptions and Distracting Influences: For example people, phone calls, noise, and so forth are periodic. Pressures related to establishing priorities, meeting deadlines, or fulfilling scheduling requirements are periodic.
• Working Environment - Very little physical effort, individuals work in comfortable positions. The tasks performed on the job, and the location of the work is such that the incumbent's exposure to accident and health hazards is unlikely.
• Modified Working Environment - Eligible on a pre-approved, temporary basis only. • Other - Possession of valid driver's license and approved insurability status by Gibault's insurance carrier required.
Performance Standards: 1. Implement the decisions of your immediate supervisor or managers within your chain of command.
2. Provide a positive role model for the students at all times through appearance, attitude, and behavior.
3. Exhibit sensitivity to the clients' cultural and socioeconomic characteristics.
4. Attend all in-services, General Staff Meetings, and meetings called by their supervisor.
5. Work harmoniously and effectively with co-workers.
6. Accomplish objectives and consistently produce satisfactory work.
7. Is faithful in reporting to work, staying on the job, and helping to meet institutional emergencies.
8. Communicate effectively, both verbally and in writing.
9. Comply with department guidelines, the Organization and Personnel Guidelines, and all expectations set by Gibault organization's administration.
10. Travel to various communities throughout the Midwest to provide aftercare services and/or visitation supervision.
11. Participate in the planning, organizing, and implementing of parent meetings for clients during placement and prior to discharge, as assigned by Aftercare/Visitation Supervisor.
12. Participate in the planning, organizing, and implementing of reintegration of client into less restrictive setting for a period of 6 months following discharge, as assigned by Aftercare/Visitation Supervisor. 13. Be available to work a flexible schedule to meet the program needs.
14. Maintain a positive relationship with other community service and program service providers.
15. Serve as a liaison between the agency and the parents of clients by generating the required correspondence and telephone calls when necessary, both in writing and in person.
16. Communicate treatment and pertinent administrative issues to Clinical Services Therapists, Supervisors, and Administration.
17. Take regular inventory of own emotional well-being. Develop and adhere to a self-care plan to promote wellness and prevent burnout.

- High school diploma or equivalent required .
- Associate's or Bachelor's degree in a related field (e.g., psychology, social work) preferred .
- Prior experience working in a behavioral health setting preferred .
- Experience with crisis intervention and behavior management techniques a plus .
- Knowledge of basic principles of mental health and behavioral disorders.
- Proficiency in implementing behavior management techniques.
- Strong crisis intervention skills.
- Excellent communication skills , both verbal and written.
- Ability to collect and document data accurately and efficiently.
- Demonstrated empathy and active listening skills.
- Ability to work effectively as part of a team.
- Experience working with specific patient populations (e.g., adolescents, individuals with substance use disorders).
- Ability to stand, walk, and sit for extended periods of time.
- Ability to lift and carry up to 25 pounds.
- Ability to physically intervene in crisis situations, including restraining patients if necessary (training provided).
- Work environment may include exposure to challenging behaviors and emotional distress.
- Must be able to work flexible hours, including evenings, weekends, and holidays, as needed.
