MSW Case Manager: Discharge & Care Coordination Lead

The University of Virginia Health System is seeking a Case Manager MSW to lead discharge planning within UVA Community Health. You will collaborate with physicians and the healthcare team to assess, plan, implement and evaluate proactive, safe discharge plans for complex patients. As part of a multidisciplinary team, you will coordinate care, ensure post-acute options are appropriate, and maintain knowledge of community resources.
