Psychiatrist - Collaborative Care
Physician Affiliate Group of New York
Overview Physician Affiliate Group of New York (PAGNY) is adding a Psychiatrist - Collaborative Care to NYC Health + Hospitals/Jacobi . Responsibilities Provide care for a diverse and challenging group of Internal Medicine patients in a Level III Patient Centered Medical Home. Work alongside a group of internal medicine physicians, nurses, nurse practitioners, social workers, and case managers providing consultative as well as direct patient care. Assess and screen patients by conducting psychiatric evaluations, interviews, medical histories, observations, and psychological testing to diagnose patients. Prescribe and administer psychiatric medications. Monitor patients to determine efficacy and potential side effects of medications. Develop a treatment plan and measure progress. Provide crisis intervention. Collaborate with a broader team of physicians, psychologists, social workers, nutritionists, and nurses to assess and adjust treatment plans.
