OASIS Workflow Coordinator (RN, PT, OT, SLP) - HomeCare

Location: 34 Ledgebrook Dr, Mansfield, CT 10179 Reporting to the Director of Clinical Services, this role ensures the accuracy, compliance, and quality of OASIS assessments and supports clinical staff with documentation, utilization, and patient care decisions. Job Responsibilities Review OASIS assessments for accuracy, completeness, and regulatory compliance; provide recommendations to ensure quality and reliability. Guide staff on clinical, documentation, and utilization decisions to improve OASIS accuracy and care quality, recommending cost‑effective corrections that support clinical and operational goals. Review visit utilization for appropriate patient care, collaborate with clinicians on needed adjustments, and report utilization concerns and financial risks to the clinical manager. Ensure timely completion of Homecare Homebase (HCHB) workflows and maintain a flexible schedule to support agency needs. Collaborate to identify documentation trends, support improvement planning, participate in quality initiatives, and help ensure compliance and accurate data submissions across all settings. Qualifications Degree from an accredited educational program leading to licensure as a Registered Nurse (RN), Physical Therapist (PT), Occupational Therapist (OT), or Speech‑Language Pathologist (SLP). Bachelor's degree in nursing, master's degree in PT, OT, or SLP preferred. 2 years in certified home health clinical required; 3 years in verified home health preferred. OASIS Certificate and/or Quality/Compliance Certificate preferred. Homecare Homebase experience preferred.
