Our focus is on providing comfort, not a cure, and quality of life, not duration. As a Home Health Aide on our Hospice team, you will care for patients where they need you most, whether at home, a nursing facility, assisted living center or other location. In this role, you can help enhance their outlook on their experience, even when their futures seem uncertain. Every day, you can make a lasting difference in both the lives of your patients and their loved ones. This position will support various counties in the Southern region which include Summit, Medina, Portage, Wayne and various others.A caregiver in this position works varying shifts, averaging 48 hours of a 6-week schedule.A caregiver who excels in this role will:Perform basic patient care activities.Assist with procedures ordered by physician, with supervision from an RN, PT, OT or ST as appropriate.Assist patients with tending to personal care, activities of daily living and transfers/transport.Report and document abnormal assessment findings or changes in physical, mental and emotional conditions to nursing staff.Assist with keeping the patient's environment clean and orderly.Maintain complete and up-to-date documentation and files at all times regarding patient condition and effectiveness and timeliness of care and interventions.Escalate change in patient condition to patient's physician as appropriate.Minimum qualifications for the ideal future caregiver include:High School Diploma or GEDState Tested Nurse Aide (STNA) Certification with the State of Ohio Nurse's Aide RegistryOne year of clinical experience as an Aide in an acute, rehab or skilled nursing settingValid Driver's License (VDL) in Ohio and proof of $100,000/$300,000 automobile insurance coverageBasic Life Support (BLS) Certification through the American Heart Association (AHA) or the American Red Cross within orientation periodPreferred qualifications for the ideal future caregiver include:Hospice and Palliative Care Nurse Assistant (CHPNA) CertificationHome Care experiencePhysical Requirements:Must be ambulatory, able to climb stairs, pull, push, and facilitate movement of the patient.For field staff only: Able to drive to patients' homes and/or facilities.Medium Work - Exerting 20 to 50 pounds of force occasionally, and/or 10 to 25 pounds of force frequently, and/or greater than negligible up to 10 pounds of force constantly to move objects.Physical Demand requirements are in excess of those for Light Work.Personal Protective Equipment:Follows standard precautions using personal protective equipment as required.Keywords: PCNA, STNA, aideThe policy of Cleveland Clinic Health System and its system hospitals (Cleveland Clinic Health System) is to provide equal opportunity to all of our caregivers and applicants for employment in our drug free environment.
