Passavant Memorial Homes is currently seeking responsible, caring, and dedicated people to fill ISO Support Service Worker positions in our Home and Community Services (HCS) program across Armstrong County. ISO Support Service Workers play a crucial role in fulfilling the mission of Passavant Memorial Homes’ HCS program by providing care and supervision to the individual in the areas of activities of daily living, as well as social and recreational activities. The ISO Support Service Worker is responsible for the safety and well‑being of the individual, as well as serving as a positive role model. The ISO Support Service Worker provides services to the individual in the individual’s home and/or in the community and uses the ISO Support Service Worker’s personal vehicle for transporting the individual, as appropriate. DUTIES AND RESPONSIBILITIES OF THE ISO SUPPORT SERVICE WORKER/CAREGIVER: Perform the tasks outlined in the individual’s Individual Plan (IP). Complete the assigned tasks in the manner in which they are requested. Perform the requested services for the individual within the units authorized within the IP. Deliver services in a manner consistent with the policies and procedures developed and provided by Passavant Memorial Homes. Provide supervision and positive role modeling at home and in the community. Complete the necessary Pre‑Service and Annual In‑Service training planned by the individual/representative and facilitated by HCS. Observe the individual’s behaviors and changes in physical condition, and report accurately. REQUIRED SKILLS AND KNOWLEDGE OF THE ISO SUPPORT SERVICE WORKER/CAREGIVER: Minimum of 18 years of age, required. Valid driver’s license, required. Current car insurance and registration, required (position requires use of personal vehicle). Successfully pass background check, required. Experience working directly with individuals with developmental disabilities, preferred.
