Service Coordinator Job DescriptionEssential Duties:Process referrals, schedule initial meetings with families, conduct screenings using standardized protocol and tools approved by AzEIP, and ensure timely delivery of servicesAssess nature of clients' situations by interviewing and reviewing personal historyEnsure proper maintenance of child files, required documentation and the AZ DES automated systemCoordinate, attend and schedule assessments; IFSP (6 months and annual), IEP, eligibility determination meetings, transition planning; notify all interested parties of meeting dates and timesCommunicate client updates to all relevant partiesFacilitate referrals to other healthcare professionals and programsDemonstrate knowledge in AzEIP Policies and Professionalism, Child Development, Family Professional Partnerships, Teaming, Assessments and Evaluation, Planning, and InterventionDemonstrate knowledge in AZ DDD/DES policiesQualifications:A Bachelor's Degree in Early Childhood, Child Development, Nursing, Psychology, Social Work, Special Education, or a Bachelor's degree in a closely related field (e.g., Psychology, Social Work, Elementary Education, or Sociology) with review of coursework and approval by AzDES/AzEIP.Candidate must successfully complete a background clearance and obtain Level One Fingerprint Card ClearanceTravel to attend meetings, training and appointmentsExcellent written and verbal communication skillsBilingual English/ Spanish for this position is a plusW2 Employee Benefits: Medical, Vision, Dental, Paid Holidays, 401k, Life Insurance, Long Term Disability, Pet Insurance, and more perks
