Registered Nurse (RN) MDS Coordinator
We are seeking a highly qualified and compassionate Registered Nurse (RN) MDS Coordinator to join our team at Jupiter, a leading provider of Rehabilitation and Nursing services in Jupiter, Florida. As an integral member of our care team, you will be responsible for coordinating the Minimum Data Set (MDS) process, ensuring timely and accurate documentation, and providing exceptional care to our patients. Registered Nurse (RN) MDS Coordinator Responsibilities:
Coordinate MDS assessments, ensuring timely and accurate completion of the MDS process
Collaborate with interdisciplinary teams to develop and implement patient care plans
Provide education and support to patients, families, and caregivers
Monitor and report on patient outcomes, quality indicators, and other metrics
Participate in quality improvement initiatives and audits
Registered Nurse (RN) MDS Coordinator Requirements:
Current Registered Nurse (RN) licensure in the state of Florida
1 years of experience as a RN or relevant experience in a healthcare setting
Resident Assessment Coordinator (RAC) Certification is preferred
Strong understanding of MDS requirements and regulations
Excellent communication, organizational, and interpersonal skills
Ability to work in a fast-paced environment and prioritize tasks effectively
