Hospice Nurse Practitioner

Job Description The Hospice Nurse Practitioner (NP) works in collaboration with the Hospice Medical Director to deliver high quality, evidence-based palliative care. By managing the symptoms of acute and chronic conditions, assisting with communication and coordination of care, and developing treatment plans, the Hospice NP is an essential leader for the Hospice Team. Care is provided mainly within patient homes and may include other settings such as outpatient clinics, hospitals, assisted living, independent living, and skilled nursing facilities. This is a full-time, salaried position. The schedule is Monday - Friday 8:00 am - 4:30 pm and includes remote on call coverage every other weekend. Responsibilities Oversee the medical care of Hospice patients in collaboration with the Hospice Medical Director. Provide consultation for Hospice patients as needed. Communicate with attending physicians as needed regarding their patient’s condition and care. Assume responsibility for the medical care of Hospice patients if the attending is unable to provide needed medical care. Complete face-to-face encounters with patients to obtain clinical data regarding the patient’s physical condition to determine the ongoing medical eligibility for Hospice. Communicate information and clinical data obtained from face-to-face encounters with the Hospice Medical Director/Hospice Physician, as necessary. Complete required attestation documenting the face-to-face encounter, the date of the visit, the clinical findings supporting the ongoing eligibility for Hospice services, and the fact these clinical findings were provided to the certifying physician. Serve as the liaison between the attending physician and the Hospice Team by obtaining/sharing information about the patient’s plan of care as needed. Complete and document the physical and psychological assessment in Electronic Medical Record based on interview, examination, and available data. Assist physicians with the assessment and treatment of patients, provide patient education and coordinate treatment with organizations for the delivery of medical care. Act as a resource to attending physicians, staff, patients, and families, as requested regarding pain and other symptom management to assure the patient/family needs are met. Develop a treatment plan which may include diagnostic testing, pharmaceutical and non-pharmaceutical interventions as well as education. Review clinical data from lab results, x-rays, scans, and pathology reports. Perform neurological, cardiac, respiratory, and other testing as indicated by the evaluation. Administer medication, injections, dressing changes and other nursing interventions and duties. Collaborate with the interdisciplinary team in establishing, implementing, and reviewing the Hospice plan of care for each patient in keeping with the Medicare Conditions of Participation as needed. Attend Patient Care Conferences and Hospice IDT meetings as needed to provide consultation and direction. Requirements Master's Degree or higher degree in Nursing Licensed nurse practitioner in New Hampshire. Certified as a Nurse Practitioner by the American Academy of Nurse Practitioners (AANP) or the American Nurses Credentialing Center (ANCC). Authority to practice and prescribe as a nurse practitioner in the state where services are provided if prescribing is permitted by state and federal law. NP must comply with all state and federal requirements to be authorized to prescribe Schedule II drugs, such as obtaining any required collaboration agreement and DEA registration number, in those states which permit the NP to prescribe Schedule II drugs. Medicare billing privileges as a nurse practitioner. Meet physician-nurse practitioner collaborative requirements specific to the state assigned to work by the Company including, if required by applicable state law, entering into a collaborative agreement and supply a copy of all active collaborating physician agreements and any required guidelines and protocols established with or by the collaborating physician within which the NP must practice. Have insurance required by state law for nurse practitioners to practice and prescribe in the state where services are provided or be eligible for such insurance requirements to be satisfied by group coverage with Company. One (1) year experience as a nurse practitioner or as facsimile specialty experience as RN. Knowledge of disease specific local coverage determinant (LCD) criteria for hospice eligibility along with knowledge of CMS Face to Face regulatory requirements.
