RN Inpatient Case Manager

Daily Responsibilities Provide case management to patients within an acute setting Responsible for quality and continuous improvement within the job scope. Responsible for all actions/responsibilities as described in company controlled documentation for this position. Contributes to and supports the corporation's quality initiatives by planning, communicating and encouraging team and individual contributions toward the corporation's quality improvement efforts. Responsible for comprehensive patient assessments through health care records, physician and patient interviews. Develops, implements, and evaluates comprehensive patient care plans. Responsible for collaboration with healthcare team to provide feedback, updates and recommendations regarding health status and plan of care for patient.
