Starting Wage Range $24.64 – $25.88 per hour Benefits No cost health, dental and vision insurance option 14.2% employer‑paid retirement plan contribution Vacation, sick and personal time Employer paid disability and life insurance Tuition reimbursement programs Qualifications Education: Minimum of a bachelor’s degree in Social Work or other relevant human service required, with licensure/limited licensure as appropriate from State of Michigan preferred, Must be eligible as a Qualified Intellectual Disability Professional (QIDP), Qualified Mental Health Professional (QMHP) and a Child Mental Health Professional (CMHP). Qualified Intellectual Disability Professional (QIDP) - Individual with specialized training (including fieldwork and/or internships associated with the academic curriculum where the student works directly with persons with intellectual or developmental disabilities as part of that experience) OR one year of experience in treating or working with a person who has intellectual disability; AND is a psychologist, physician, educator with a degree in education from an accredited program, social worker, physical therapist, occupational therapist, speech-language pathologist, audiologist, behavior analyst, registered nurse, registered dietician, therapeutic recreation specialist, a licensed/limited-licensed professional counselor, OR a human services professional with at least a bachelor’s degree in a human services field. Qualified Mental Health Professional (QMHP) - Individual with specialized training (including fieldwork and/or internships associated with the academic curriculum where the student works directly with persons receiving mental health services as part of that experience) OR one year of experience in treating or working with a person who has mental illness; AND is a psychologist, physician, educator with a degree in education from an accredited program, social worker, physical therapist, occupational therapist, speech-language pathologist, audiologist, behavior analyst, registered nurse, therapeutic recreation specialist, licensed/limited-licensed professional counselor, licensed or limited licensed marriage and family therapist, a licensed physician’s assistant, OR a human services professional with at least a bachelor’s degree in a human services field. Child Mental Health Professional (CMHP) - Individual with specialized training and one year of experience in the examination, evaluation, and treatment of minors and their families and who is a physician, psychologist, licensed or limited-licensed master’s social worker, licensed or limited-licensed professional counselor, or registered nurse; or an individual with at least a bachelor’s degree in a mental health-related field from an accredited school who is trained and has three years supervised experience in the examination, evaluation, and treatment of minors and their families; or an individual with at least a master’s degree in a mental health-related field from an accredited school who is trained and has one year of experience in the examination, evaluation and treatment of minors and their families. For the BHT/ABA services individuals must be a BCBA or BCaBA or Psychologist working within their scope of practice with extensive knowledge and training on behavior analysis and BCBA certified by 9/30/2020. Experience One year working with the SED or IDD/DD children population and meet state requirements as a Qualified Mental Health Professional or Qualified Intellectual Disability Professional. Proficient computer skills including Office 365. Other Ability to interact with physicians and other healthcare providers in a professional manner. Working knowledge of physical and mental health, chronic health conditions, and behavioral health medications. Summary of Responsibilities The Family Support Specialist (FSS) assesses, plans, links, advocates, coordinates, and monitors to assist children or young adults still in school in gaining access to needed health and dental services, financial assistance and benefits, housing, education, social services, and other natural supports using a family driven, youth guided planning process. Essential Job Functions Assures that the person‑centered planning process takes place and that it results in the individual plan of service, and meets direct and indirect service activity expectations Assures that the plan of service identifies what services and supports will be provided, based upon medical necessity, who will provide them, and how the FSS will monitor the services and supports identified under each goal and objective Oversees the implementation of the individual plan of service, including supporting the child and family’s dreams, goals, and desires for optimizing independence, promoting recovery, and assisting in the development and maintenance of natural supports Assures the participation of the child and family on an ongoing basis in discussions of her plans, goals, and status Identifies and addresses gaps in service provision Coordinates the child’s services and supports with all providers, making referrals, and advocating for the child and family Assisting the child and family to access programs that provide financial, medical, and other assistance such as Home Help and Transportation services and Medicaid Enrollment and Spend‑downs Assures coordination with the child’s primary and other health care providers to assure continuity of care Coordinates and assists the child in crisis intervention and discharge planning, and community supports after hospitalization Facilitates the transition process (e.g., from inpatient to community services, school to work, dependent to independent living), including arrangements for follow‑up services Assists the child and family with crisis planning. Participates as member of Children’s Crisis Mobile Team Identifies the process for after‑hours contact Monitors and ensures that applicable laws, rules, policies, and procedures are followed Reports cases of alleged abuse or neglect to agency Recipient rights Officer and/or with Michigan Department of Health and Human Services Protective Services Workers or Licensing Consultant as prescribed by law Provides accurate and timely documentation for the clinical records and data systems, in accordance with Medicaid and agency requirements May assist in transporting and accompanying individuals and parents to medical appointments, evaluations, etc. Participates in meetings and other related duties as directed by the supervisor Complies with agency requirements for continuing education, peer review, credentialing/privileging, and QIDP and/or QMHP requirements May assist, arrange, or provide in‑service training programs for consumers, direct support workers, foster care providers, parent, guardians, and interested community members or civic organizations Performs other duties as assigned by supervisor Physical Requirements Ability to walk, bend, stand, sit, lift to 25 pounds with or without assistance, stretch/reach, hear, see, hand/finger dexterity and drive a vehicle. Position requires physical ability to complete and maintain certification in de‑escalation techniques, which will be a training class provided to the job incumbent. Other Requirements Must have own phone and internet for communication, reliable access to messages, and multi‑factoring capabilities. Must be able to complete and remain current in initial and ongoing training requirements. Have reliable transportation and be able to drive a company vehicle. Must also possess a valid Michigan driver’s license and have a satisfactory driving record. Ability to work within guidelines of the code of conduct, regulatory compliance plan and personnel policies. Supports a philosophy of service delivery that is recovery‑based, person centered and culturally competent. Skilled in planning, organizing, and prioritizing. Possesses high ethical standards, trustworthiness, and is a person of integrity. Maintains appropriate professional image; able to maintain composure and effectiveness under pressure and in changing conditions; able to accept feedback as it relates to job performance and responsibilities. Possess good judgement and problem solving within a team environment. Excellent interpersonal, oral and written communication skills. Ability to facilitate a group. Able to effectively present clinical information to psychiatry or to supervision. Ability to negotiate conflict and maintain constructive working relationships with people at all levels of the organization and with community stakeholders, families, and clients. Has knowledge of standards, policies and procedures regarding rights, confidentiality, Mental health code, HIPPA, code of ethics etc. Demonstrates basic computer skills required for clinical documentation, communication and data entry. Ability to visit clients in their residence, educational or employment setting. Irregular hours will be necessary periodically. Embraces the c1C/S Vision Ability to interact with physicians and other healthcare providers in a professional manner. Working knowledge of physical health, chronic health conditions, and behavioral health medications.
