The Discharge Planner must be able to demonstrate competency in the criteria listed in this job description. It is imperative for the Clinical Case Coordinator to have outstanding organizational skills and the ability to constantly multi-task.

Qualifications

Bachelor's Degree in the human services field (counseling, psychology, social work) required, Master's degree preferred. Previous professional experience in a behavioral health setting preferred.
