Assess and evaluate patients in both inpatient and outpatient settings, creating tailored rehabilitation plans. Deliver physical therapy treatments that emphasize mobility, strength enhancement, balance restoration, and functional improvement. Work collaboratively with an interdisciplinary team, including nurses, physicians, and case managers to achieve optimal patient outcomes. Educate patients and their families on therapeutic goals, discharge strategies, and home exercise regimens. Keep detailed records of assessments and progress according to established guidelines. Engage in care team meetings and interdisciplinary conferences to ensure unified care pathways.

Doctorate in Physical Therapy (DPT) or an equivalent credential from an accredited institution. Valid state licensure as a Physical Therapist. CPR certification preferred. Additional certifications in specialties (neurological, geriatric medicine) are welcome but not mandatory.

Prior experience in a similar role is highly desirable. Ability to work with patients facing complex medical conditions and mobility challenges. Excellent communication abilities and strong teamwork mindset. Proficient in electronic medical records and clinical documentation systems.
