Description

Conduct patient evaluations in inpatient or outpatient settings and create personalized treatment plans
Implement physical therapy interventions focusing on mobility, strength, balance, and functional outcomes
Collaborate closely with nurses, physicians, case managers, and other rehabilitation professionals to optimize patient outcomes
Instruct patients and their families on therapeutic objectives, discharge planning, and at-home exercise protocols
Document all evaluations, treatments, and patient progress in line with facility and regulatory standards
Engage in care team discussions and interdisciplinary meetings to guarantee comprehensive patient care
Education
~ Doctorate in Physical Therapy (DPT) or equivalent from an accredited program

Certifications
State licensure as a Physical Therapist
CPR certification is preferred
