Behavioral Health Case Manager I

Overview Behavioral Health Case Manager at Adelante Healthcare. Full-time position located in Mesa, AZ. Education: HS Diploma from an accredited school. Bachelor’s/Associate’s degree preferred but not required per the original content. Responsible for managing an assigned caseload, assessing client needs, developing and implementing service plans, and coordinating with internal and external resources to meet client service needs. Responsibilities Participate in screening new service requests within the center to ensure individuals are adequately served according to their needs and in accordance with policies and procedures. Participate in regular interdisciplinary staff meetings. Accept newly assigned cases where the diagnostic intake has been completed by the Intake Specialist; complete case management assessment for all clients entering caseload. Develop appropriate treatment/service plans with clients to implement services and build effective alliances. Provide direct client services to individuals and/or family members. Services may include case management, skills training, group skills training, health promotion, transportation, supportive employment services, counseling with clinical oversight, and others depending on client need. Services may be provided in office, home, school, or community settings. Provide ongoing supportive and/or case management functions to assist clients in achieving goals identified in the service plan. Conduct face-to-face reviews with clients and/or relevant staff, report progress, and adjust the service plan as necessary to achieve goals. Assist supervisory staff with tracking clients and generating reports to support quality of care. Document all service contacts promptly, including interviews, collateral contacts, and correspondence; maintain case records in accordance with internal and regulatory standards. Maintain communication with the consulting psychiatrist and Behavioral Health team regarding medication compliance, side effects, and medication changes; alert the team to changes suggesting potential decompensation requiring intervention. Perform other duties as assigned by the supervisor in line with Adelante Health Care policies and procedures. Qualifications Essential Skills and Experience: High school diploma or GED required; Associate’s Degree preferred. Minimum of one year of experience in Behavioral Health preferred. Excellent customer service and computer skills. Thorough knowledge of current versions of DSM and ICD preferred. Ability to complete a Behavioral Health Assessment with appropriate clinical oversight. Ability to provide effective and respectful quality care responsive to diverse cultural beliefs and practices. CPR and AED certification for Health Care Professionals (American Heart Association/Red Cross) via approved courses. Bilingual English/Spanish strongly preferred. Valid Arizona driver's license. Valid Level One Fingerprint Clearance Card issued by Arizona Department of Public Safety. Non-Essential Skills and Experience Experience with community resources and agencies preferred. Additional Duties Other duties as requested or assigned. Adherence to Adelante Healthcare’s Compliance Program and Standards of Conduct; compliance is part of performance evaluations. Physical Demands and Work Environment The role may require sitting for long periods, standing, walking, using hands, reaching, climbing stairs, balancing, kneeling, bending, and occasional lifting up to 50 pounds with assistance. Vision requirements include close, distance, color, peripheral, depth perception, and focus adjustment. Travel between clinic sites may be required, with a moderate noise level and potential exposure to health hazards.
