Responsibilities Provide skilled nursing care in patients’ homes under the direction of an RN/physician
Monitor and document patient condition and response to treatment
Administer medications and treatments as ordered
Educate patients and families on care plans and disease management
Assist with coordination of care among interdisciplinary team members
Complete timely and accurate clinical documentation
Qualifications Active Oklahoma LPN License
Current BLS/CPR Certification
Valid Driver’s License and Reliable Transportation
Home Health Experience Preferred
Strong Clinical Assessment and Communication Skills
Ability to work independently in a personal setting
