Pediatric Nurse Practitioner

Pediatric Nurse PractitionerWe are seeking a dedicated and experienced Pediatric Nurse Practitioner (CNP) to join our clinical team. This role works as part of the clinical provider team, delivering a wide range of direct primary care services, including patient assessment, diagnosis, treatment implementation, evaluation, and ongoing monitoring of care plans. The ideal candidate will collaborate with physicians and healthcare staff to provide high-quality, patient-centered care and drive positive health outcomes.Key Responsibilities:Provide high-quality patient care through the diagnosis and treatment of acute and chronic conditionsObtain and review patient histories (medical, developmental, social, and family) and perform comprehensive physical examsOrder, perform, and interpret diagnostic tests and preventive screeningsDevelop and manage treatment plans, including prescribing medications and educating patients on conditions and carePromote preventive care through immunizations, screenings, and patient education on health, nutrition, safety, and disease preventionDocument all patient encounters accurately, including history, exam findings, diagnoses, and care plansReview and maintain patient records, including labs, imaging, medications, and immunization statusCoordinate care by facilitating referrals and collaborating with healthcare team membersMonitor patient progress, follow up on test results, and manage high-risk patient population.Qualifications:Master's or Doctoral degree in NursingCertified Nurse Practitioner licensed in the State of OhioWork Experience:Minimum of 5 years of experience in family medical practiceFNP preferred, with a strong desire to work with children.Experience in a physician office, ambulatory care, or clinical health setting preferredQualifications:Board certification as a Nurse PractitionerActive DEA license (or ability to obtain)Strong clinical decision-making and communication skillsCommitment to patient-centered carePreferred Qualifications:Experience in primary care, family medicine, or community healthBilingual skills (if applicable)Familiarity with electronic health record (EHR) systemsEqual Employment Opportunity/Drug-Free Workplace: The HealthCare Connection is focused on creating a community that promotes dignity and respect for employees, patients and other community members.
