RN Hospice Case Manager

Suncoast Hospice, a part of Empath Health, is seeking a caring and motivated RN Case Manager to work with us in Pinellas county while providing exceptional care on the following schedule: Monday-Friday 8-5.

What you'll do:

• Under the direction of a licensed physician, manage discomfort and provide symptom relief using specialized nursing skills related to palliative and end-of-life care.
• Provide initial and ongoing assessment of the impact of the terminal diagnosis on the patient's physical, functional, psychosocial, and environmental needs and activities of daily living.
• In collaboration with the other members of the Interdisciplinary Group (IDG); develop, implement, and update the individualized plan of care, initiate appropriate preventive and rehabilitative procedures and initiate referrals to other services; request complimentary services, as needed.
• Educate patient/resident, family and caregivers regarding disease process and decline, palliative interventions care giving, dying process and safety practices.

What you'll need:

• Graduate of approved nursing program and current unrestricted Florida RN license.
• Valid and current CPR certification required.
• Minimum one year experience in an acute-care hospital environment providing direct patient care; previous experience as a Hospice RN preferred.
• Bilingual preferred.
• CHPN certification is strongly encouraged within three years of employment.
• BSN or MSN preferred.
