Hospice RN Case Manager Lead End-of-Life Care
Gentiva Hospice
Gentiva Hospice seeks a dedicated RN Case Manager to deliver skilled nursing care at patients' homes, assisted living facilities, or other home settings. You will assess, plan, and document care while coordinating with physicians, social workers, and other team members to ensure compassionate, comprehensive end-of-life support.
You will educate families, manage symptoms, and adapt plans of care as conditions change, maintaining compliance and accurate records throughout.
