They will then be on night call from 5pm-8:30am the next day (billed at the night call rate). Weekends will be 24hr call coverage. We would like to note they may not be in clinic all 5 days during the dates listed but it is a possibility. • Dates of Need: See above • Shift Times: 8:30am-5pm , 5pm-8:30am • Lunch N/A • Provider Min Shifts Required: Please ensure bids reflect the following: • Guaranteed Hours per Day: 8 • Hours Worked for OT to apply: 9 hours per day • Rates to be Filled Out on Bid: Clinic & Call: Regular, Orientation, Overtime, Holiday, Night Call, 24 Hour Call, Call-Back. ________________________________________________________________ CREDENTIALING • Current CV (must be uploaded to bid) • License: Active or IMLC • DEA Required: Yes • Boards: Must be Board Certified or truly Board Eligible within 3-5 years of residency is an option but we are looking at BC. • Skills/Certs: Will see complex spine cases, No airway certifications required • Other: FLURO certification is required (they can have the temp FLURO) We will need providers to submit during the privileging process 100 cases within the past 24 months specific to the privileges being requested. PLEASE ENSURE AN UPDATED CV WITH ALL HOSPITAL AFFILIATIONS IS UPLOADED DURING PRESENTATION. Attached is a new privileging form that will set expectations for what is needed once a provider is confirmed. Please consider starting to ensure your provider has all of these items to ensure they can qualify for privileges at the time of confirmation. o All HBS specialties require 2 years of current Hospital clinical activity in the specialty. o Gaps greater than 3 months must be explained. o Include current number of case logs upon presentation for surgical specialties.
