Responsibilities

• Conduct comprehensive PT evaluations and develop individualized treatment plans for SNF residents.
• Deliver evidence-based interventions to improve mobility, strength, balance, and functional independence.
• Document assessments, progress notes, and discharge summaries in the patient record per facility guidelines.
• Perform gait training, therapeutic exercise, and modality use as appropriate to patient needs.
• Collaborate with nursing, OT, speech-language pathology, and medical staff to optimize patient outcomes.
• Educate patients and families on home exercise programs, safety, fall prevention, and activity modification.

Requirements

• Current physical therapy license valid in Maryland.
• Doctor of Physical Therapy (DPT) degree or equivalent.
• Eligibility to practice in a skilled nursing facility setting.
• Prior SNF or geriatric rehabilitation experience preferred.Strong communication and documentation skills, with ability to work as part of a multidisciplinary team.
