Overview Physical Therapist – Home Health Baker, Montana, United States Start date: June 22, 2026 Duration: 13 weeks Weekly pay: $2,444–$2,594 per week Guaranteed hours: 40 per week Shift: 8:00 am–5:00 pm, Monday–Friday Setting: Outpatient department within a hospital (home health focus with outpatient emphasis) Key Highlights Weekly pay: $2,444–$2,594 per week Guaranteed hours: 40 hours per week Contract duration: 13 weeks Shift: 8:00 am–5:00 pm, Monday–Friday Start date: June 22, 2026 Extension opportunities: potential for longer engagement Responsibilities Deliver individualized physical therapy services in an outpatient hospital setting, including assessment, treatment planning, implementation, and progress monitoring Develop and implement customized treatment plans to maximize functional outcomes and return to activity Educate patients and families on exercise programs, self-management, and safety to support ongoing progress Reassess patient progress regularly and adjust plans to meet functional goals Document evaluations, treatment plans, progress notes, and outcomes clearly in the medical record Collaborate with interdisciplinary team members to coordinate care and ensure consistent communication Requirements Availability to work a 40-hour week, Monday through Friday, 8:00 am–5:00 pm Ability to perform PT duties in an outpatient hospital setting with occasional LTC/Skilled Swing tasks Strong communication and documentation skillsComfort working within a collaborative multidisciplinary team Eligibility to participate in a 13-week travel assignment and to travel to Baker, MT for the duration of the contract Benefits and Support 13-week travel assignment with potential extension Guaranteed hours Competitive weekly pay within the stated range Housing: not included; a list of housing options can be provided Assignment support and coordination through Healthforce Healthcare LLC #J-18808-Ljbffr HealthForce Healthcare
