This role offers the unique opportunity to start as a Visiting RN and transition into a Case Manager position, allowing you to utilize your clinical expertise to ensure continuity of care and enhance patient outcomes.

As a Registered Nurse Case Manager, you will:

• Provide skilled nursing and coordinate care for patients and their families in a residential setting through a team approach.
• Observe, assess, evaluate, and plan interventions based on patients' physical and psychosocial statuses.
• Oversee care provided by Home Health Aides and CNAs, ensuring compliance with the plan of care.
• Ensure continuity of care through effective communication and case conferences with other disciplines and community health agencies.
• Maintain accurate and complete patient records in accordance with agency standards and OASIS requirements.
• Participate in team scheduling coordination and on-call rotation as necessary to meet patient care needs.
• Engage in quality improvement activities and attend in-service training as assigned.

Requirements:

• Current RN license in the State of Montana (BSN preferred).
• Valid driver's license, appropriate auto insurance, and reliable transportation.
• Need 2 years experience in setting.
• Experience in acute care, home care, or hospice, including clinical skills such as venipuncture, catheterization, wound care, and/or parenteral therapy preferred.
• HCHB experience strongly preferred.
