Requirements: BLS required; Acute Hospital experience required; 2 years of experience required

Responsibilities:

• Manage discharge planning and care coordination for patients in an acute rehabilitation setting
• Collaborate with interdisciplinary teams to develop comprehensive care plans
• Facilitate patient transitions and ensure timely discharges
• Document patient progress and coordinate services with healthcare providers
• Support patient education and engagement during hospitalization
