Job Summary:

Provides outpatient and home health physical therapy services to patients, focusing on improving mobility, strength, and overall function. Ensures personalized care plans are effectively implemented in various community settings.

Responsibilities:

• 
  Evaluate patients' physical conditions and develop individualized treatment plans.
  
  
• 
  Deliver therapy services in home health and outpatient environments.
  
  
• 
  Record and document patient progress accurately and timely.
  
  
• 
  Maintain compliance with healthcare regulations and documentation standards.
  
  
• 
  Communicate effectively with patients, families, and healthcare team members.
  
  
• 
  Manage transportation and travel between patient locations within assigned territory.
  
  

Qualifications:

• 
  Valid Hawaii license (in progress accepted) required at application.
  
  
• 
  Minimum of 1 year of Home Health experience required; outpatient experience preferred.
  
  
• 
  Proven competency in OASIS documentation is preferred.
  
  
• 
  Current BLS certification from AHA required.
  
  
• 
  Must hold an active drivers license and own transportation.
  
  
• 
  Ability to work independently in various community settings.
