Responsibilities Provide outpatient rehabilitation services in an in‑home setting for the older adult population, evaluating and treating patients in the most functional environment. Assess therapy needs, develop and deliver individualized physical therapy programs, and refer to other disciplines as needed. Educate and train patients and caregivers to promote independence and reduce pathology. Supervise Physical Therapy Assistants, temporary licensees, students, and support personnel in accordance with state licensure requirements. Maintain timely clinical and billing documentation, attend patient care and staffing conferences, and comply with department schedule. Perform screenings, examinations, diagnosis, and plan of care, including functional, therapeutic exercise, manual therapy, airway clearance techniques, and more. Design adaptive devices, provide case management in home and community settings, and incorporate health and wellness programming. Order supplies, maintain equipment, and promote a safe environment. Adjust treatment plans to meet individual patient needs and ensure highest quality service. Qualifications Valid Physical Therapy license in state(s) of practice, or proof of license eligibility. Master’s degree in Physical Therapy, or Master’s in a related field plus a Bachelor’s in Physical Therapy. Alternate requirement: Bachelor’s degree in Physical Therapy plus 5 years of progressively responsible PT experience. U.S. equivalent evaluation for degrees earned outside the U.S. Valid driver’s license or reliable transportation to patient residences.
