Registered Nurse (RN) Case Manager - Hospice

Job Summary:

Amara Hospice Care is seeking a compassionate and experienced Registered Nurse (RN) Case Manager to join our team in Metro Detroit and surrounding areas. As an RN Case Manager, you will play a vital role in providing high-quality, patient-centered care to individuals with life-limiting illnesses. You will work closely with patients, families, and healthcare teams to develop and implement individualized care plans, ensuring that patients receive the most comprehensive and compassionate care possible.

Responsibilities:

• Complete initial assessments of patients and families to determine hospice care needs

• Regularly re-evaluate patient nursing needs and update care plans accordingly

• Collaborate with healthcare teams, patients, and families to develop and implement individualized care plans

• Counsel patients and families on end-of-life care and provide emotional support

• Initiate preventative nursing procedures and communicate patient needs to physicians

• Supervise Hospice Aides and LPNs

• Participate in On-Call rotation

• Provide care to hospice patients in their homes and facilities, Monday through Friday, 8:30am to 4:30pm

Requirements:

• Current Michigan RN License in good standing

• Hospice experience preferred

• Reliable transportation and insurance

• Ability to travel to patient locations

• Monday through Friday, 8:30am to 4:30pm schedule

Salary: $82,000 - $90,000 annually
