Hospice RN Case Manager - Patient-Centered Home Care
Crossbridge Hospice
Crossbridge Hospice is seeking an RN Case Manager in Troy, MI to provide end-of-life care in patients' homes or community settings, with intermittent skilled nursing services and supervision of hospice aides. The role requires compassion, strong clinical judgment, and excellent communication as you support patients and families through highly sensitive and meaningful experiences. Schedule: Mon-Fri 8am-5pm with on-call as needed; you will coordinate care, maintain notes, and collaborate with the #J-18808-Ljbffr Crossbridge Hospice
