Deliver Patient-Centered Care Beyond the Clinic Walls \n

Bellin Health is seeking a  Physician to join our innovative  My Home My Care program , serving patients across  Escanaba, Iron Mountain, Peshtigo, Green Bay, and surrounding communities . This unique role offers the opportunity to provide  high-quality, relationship-based care directly in nursing homes, assisted living facilities, and select home settings .

This is an ideal opportunity for a physician who values  autonomy, meaningful patient connections, and a proactive approach to care delivery outside of the traditional clinic setting.

• Full-time (1.0 FTE), Open to part-time

  • 80% Nursing Home
  \n
  • 20% Assisted Living/home
  \n
\n
• Mobile, community-based care model
\n
• Provide  on-site care in partner facilities and select patient homes
\n
• Strong interdisciplinary team support
\n

\n Practice Details \n

• \n

  Deliver  transitional, acute, and routine care in:

  \n
  \n
  • Nursing homes
  \n
  • Assisted living facilities
  \n
  • Select private residences (case-by-case)
  \n
\n
• \n

  Practice operates as a  mobile clinic model , bringing care directly to patients

\n
• \n

  Focus on managing  chronic conditions and complex patient needs

\n
• \n

  Collaborate with a  multidisciplinary care team to provide proactive, coordinated care

\n

This model enhances patient outcomes by eliminating barriers to access and ensuring continuity of care in familiar environments.

• 2-year compensation guarantee

  • 264 hours annually after guarantee
