Schedule

• Sunday: 10:00 AM – 9:00 PM
• Monday: 3:00 PM – 9:00 PM
• Thursday: 3:00 PM – 9:00 PM

Position Summary

The Direct Support Professional (DSP) – Home and Community provides individualized support to people with disabilities, focusing on self-help, socialization, and independent living skills. Services are provided in the individual's home and throughout their community, aligned with person-centered goals and outcomes.

Key Responsibilities

• Support individuals in achieving goals through person-centered approaches.
• Maintain a balance between health and safety and individual independence.
• Assist with home and community-based activities that promote growth and engagement.
• Maintain professional relationships with families and team members.
• Attend team meetings and communicate regularly with supervisors.
• Complete all required trainings and comply with incident reporting standards.
• Use a smart device for documentation and communication as needed.

Qualifications

• High School Diploma or GED
• Ability to complete one-person physical transfers
• Valid Pennsylvania driver's license
• Must meet Act 33/34 clearance requirements
