Tryfacta is seeking a Registered Nurse - Case Manager for our client in Santa Rosa, CA 95405. This is a 13 week contract assignment. If you meet the qualifications listed below and are interested, please apply now!
Job Title: Registered Nurse - Case Manager
Duration: 13 Weeks
Shift: Day 5x8-Hour (08:00 - 16:30)
Location: Santa Rosa, CA 95405
Requirements:
Charting System: EPIC
Start Date: 06/15/2026
First time Traveler: Yes
Locals accepted: Yes, with less BR
Years of experience: 2 years of experience
Special Procedures/Unit Details:
• Ability to organize and participate in family meetings.
• Participate in the development of the treatment plan.
• Experience with HMOs, IPAs, etc.
• Competent with arranging DME, SNF and ambulance transport. Must be able to perform this task independently.
Float between like units? Yes, based on need
RTO Restrictions: Please include all RTO at the time of submittal
Charting: Epic

Patient Age Groups: Adults, Geriatrics
