Vocational Specialist - ACT/FACT

Job Overview The ACT/FACT Vocational Specialist is a member of a multidisciplinary Assertive Community Treatment Team. Responsibilities include providing supportive services—outreach, monitoring, assessment, service planning, support network building, family consultation, skill training, and community resource referrals—to assist ACT participants in attending school, volunteering, and working. Job Details Full-Time, 40 hours/week. Rotating schedule may include weekend, evening, and holidays. On‑call crisis services as needed. Responsibilities Meet with ACT participants in homes or community venues to provide in vivo services that improve quality of life and maintain community tenure. Assist with activities of daily living, budget planning and money management, stable housing maintenance, accessing benefits, meal planning, grocery shopping, reliable transportation, side‑by‑side coaching, and training. Transport and accompany clients to scheduled community activities or appointments. Provide medication service—including delivery and observation of ingestion—under direction of team psychiatrist, nursing staff, and Team Leader. Coordinate services for an assigned group of clients in collaboration with other ACT staff. Develop, write, implement, evaluate, and revise treatment goals. Provide individual and supportive counseling for symptom management. Offer on‑call crisis services on a rotating schedule. Document all services promptly using the electronic health record. Cross‑train with other ACT specialties to provide additional services. Develop relationships with community employers and assist participants in matching with jobs, offering on‑the‑job support and coaching per the Supported Employment model. Assess participants’ educational and work history, set vocational engagement goals, and develop plans to achieve those goals. Facilitate educational and support groups addressing vocational and educational goals. Provide primary support to ACT participants and cross‑train the team to assist with housing‑related supports to promote mental health stability and reduce forensic/hospital recidivism. Perform other duties as assigned. Qualifications Education: Bachelor’s degree with major coursework in criminal justice, sociology, social work, psychology, counseling or related field. Degree in rehabilitation counseling preferred. Experience: Minimum two years of direct‑service experience with individuals with mental health concerns. Preferred experience with forensic involvement and/or co‑occurring substance use disorders. Skills: English fluency in speech, reading, and writing. Additional fluency in a second language (e.g., Spanish) preferred. Basic mathematical reasoning. Clean driving record and an active driver’s license. Physical and Sensory Requirements Heavy work: lift and/or carry up to 50 lbs., sit, stand and/or walk at least 6 hours in an 8‑hour day. Stooping, crouching, grasping and fine manipulation required at varying frequencies. Good physical and mental health, visual and auditory acuity required to provide comprehensive care.
