This full-time Physical Therapist position may have potential to support nearby sister facilities, serving adult and geriatric residents in a post-acute settings. As a key member of an interdisciplinary rehab team, you will help residents improve mobility, strength, balance, and pain management to maximize safety, independence, and quality of life.ResponsibilitiesEvaluate, diagnose, and treat adult and geriatric residents with mobility, strength, balance, gait, and pain impairments in a SNF/LTC environment.Develop and implement individualized, goal-driven treatment plans in collaboration with nursing, OT, SLP, and medical staff.Complete timely, accurate documentation of evaluations, daily treatments, progress notes, and discharge plans per facility, payer, and regulatory standards.Educate residents, families, and staff on therapeutic exercises, mobility aids, fall prevention, and home safety.Participate in care conferences, QI initiatives, and program development to support strong clinical and operational outcomes.QualificationsCurrent, unrestricted Physical Therapist license in the state of practice (or eligibility prior to start).Prior experience in skilled nursing, post-acute rehab, or long-term care strongly preferred.Excellent communication, documentation, and collaboration skills; resident-centered, outcomes-focused approach.Compensation and BenefitsThe hiring facility offers a competitive hourly rate, plus a comprehensive benefits package.
