About This Role The LTSS Service Care Manager - Behavioral Health focuses on developing and coordinating holistic care management activities for populations with significant mental/behavioral health needs. This role is ideal for professionals who are passionate about improving healthcare outcomes for members with complex needs. Highlights Pay: $27.02 - $48.55 per hour Location: Remote, TX Schedule: Full Time, Day shift Residency: Must reside in Texas Population: Members with significant mental/behavioral health needs Work Style: Hybrid position with in-person member engagement and remote work What You'll Do Evaluate the needs of complex and high-risk members with mental/behavioral health needs. Act as liaison and member advocate between the member/family, physician, and facilities/agencies. Support members with mental/behavioral health needs, including major depression and substance use disorder. Perform frequent home and/or site visits (up to 75% monthly travel) to assess member needs. Provide education to long-term care members and their families on healthcare topics. Educate on and coordinate community resources, including medical and social services. Ensure appropriate referrals based on individual member needs. Maintain accurate documentation in the electronic care management system. Provide feedback to leadership on opportunities to improve care quality. Comply with all policies and standards. Requirements Education: Master's degree in Mental Health or Social Work, or Graduate from an Accredited School of Nursing Experience: 2 – 4 years of related behavioral health/mental health experience Preferred: 5+ years of Behavioral Health Case Management experience Licenses & Certifications: One of the following licenses required: LCSW, LMFT, LMHC, LPC, or RN with direct Behavioral Health experience Benefits Competitive pay Health insurance 401K and stock purchase plans Tuition reimbursement Paid time off plus holidays Flexible work schedules About Centene Centene is a leading healthcare provider committed to delivering exceptional patient care.
