Home Health Physical Therapist (PT) – Full-time Presidential Home Health Care is currently seeking a Registered Physical Therapist in the South Shore of Massachusetts to Greater Boston area. Responsibilities Provide skilled Physical Therapy services on an intermittent basis to patients in their homes according to physician orders. Conduct initial and ongoing comprehensive assessments of patients’ needs. Develop and implement treatment plans in consultation and approval of the physician. Assist the physician in evaluating patients by applying diagnostic and prognostic muscle, nerve, joint, and functional ability tests. Treat patients to relieve pain, restore or improve function, and maintain maximum performance using patient‑centric care plans that include therapeutic exercises and other interventions. Consult and brief staff regarding the established plan. Teach, supervise, and counsel patients and families in the physical therapy program. Instruct patients in the care and use of wheelchairs, braces, crutches, canes, and prosthetic or orthotic devices. Maintain timely and quality documentation of patient evaluation data, treatments, and responses to therapy interventions, and prepare periodic assessment summaries. Supervise Physical Therapy Assistants and paraprofessionals as per agency policy. Participate with staff, patients, and physicians in discharge planning activities and complete a Physical Therapy Discharge Summary. Qualifications Graduate of a Physical Therapy school approved by the Council of Medical Education of the American Medical Association in collaboration with the American Physical Therapy Association. Licensed or registered by the State of Massachusetts. Current CPR certification. New graduates will undergo an agency‑sponsored training program.
