Treatment & Triage Nurse

The Treatment and Triage Nurse provides direct patient care to adults with complex, chronic disease in an Ambulatory ICU Patient Centered Medical Home setting.

The role functions as a part of a large, integrated care team alongside the Primary Care Provider, Social Work, Nurse Care Management, Behavioral Health and others with the goal of providing whole-person care in the outpatient setting in order to avoid Emergency Room and hospital utilization by supporting our members to overcome their multiple barriers to care and self-management.

Education & Experience

• Registered Nurse with 5 + years related experience in hospital or ED care of complex adult patients with multiple diseases
• Skilled with IV placement and phlebotomy
• Knowledge and experience using Electronic Medical Records and ability to analyze and leverage their reporting capabilities
